Description
This grant supports research activities that aim to improve the management of anticoagulant therapy and develop expertise in implementation science within cardiovascular health services. The project focuses on implementation of evidence-based clinical care for anticoagulant medications, outpatient management of low-risk pulmonary embolism, use of electronic health records and clinical pharmacists to improve prescribing practices, and mentorship and training in implementation science methodology.
