Description
This project aims to conduct a pragmatic trial testing the implementation and effectiveness of a survivorship care delivery intervention across eight Community Health Center sites in Texas. The study will be guided by theory-based models to maximize successful implementation and sustainability, generating lessons to scale this primary care-based model to improve cancer survivorship care in community health settings.
