Description

The project aims to implement nurse-led telephonic transition and palliative care intervention for people living with dementia (PLWD) to reduce emergency department (ED) visits and hospitalizations, and improve quality of life. The intervention will follow PLWD for 6 months, focusing on care transitions, managing behavioral and psychological symptoms of dementia (BPSD), care partner strain, and serious illness conversations. The study will optimize the intervention through user-centered design, conduct a 3-arm cluster randomized trial across 40 EDs, and analyze implementation factors and outcomes to inform scalable dyad-centered care.
