Description

This project aims to improve the outcomes of sepsis survivors through an intervention called I-TRANSFER, which enhances the identification and communication of sepsis survivors to home health care (HHC) services. The study tests the effectiveness of the intervention in reducing 30-day rehospitalization and emergency department visits among sepsis survivors receiving home health care. It also investigates the implementation process using a pragmatic, Type 1 hybrid, stepped wedge randomized trial involving dyads of acute and HHC stakeholders.
