Description
This project aims to reduce frequent utilization of acute care among homeless veterans by integrating data analytics with Peer Specialists trained in Whole Health Coaching (PS-WHC). The research will test whether this innovative model of care reduces homeless Veterans' reliance on acute care services through a randomized controlled trial at the Palo Alto and Bedford VA facilities. The intervention includes 12 sessions of Whole Health Coaching over 12 weeks and evaluates impact on acute care utilization, health outcomes, patient activation, and support service access.
