Description

The project aims to identify the proportion of potentially preventable hospitalizations (PPH) in persons living with dementia, understand mechanisms leading to hospitalization, and develop strategies to reduce PPH within the Medicare-funded dementia care coordination program GUIDE. The research will analyze 700 hospitalizations and 100 likely PPH cases using electronic medical record data, physician adjudication, caregiver interviews, and clinician surveys.
