Description

This project aims to improve quality of life and well-being among breast and prostate cancer survivors by targeting anti-racism training in clinical settings and using universal social determinants of health screening and referral to address such needs. Community health workers utilizing health information exchange in DC will improve community-clinical linkages and decrease social isolation among cancer survivors. The long-term goal is to decrease cancer outcome disparities in the Washington DC region.
