Description

This study tests the effectiveness of the I-TRANSFER intervention which improves identification and communication of the sepsis survivor to home health care, provides early attention to prevent poor outcomes, and provides guidance on implementation determinants from a diverse sample of hospitals and home health agencies. The results have the potential to inform clinical guidelines in home health care nationally and improve recovery of sepsis survivors.
