GES - INVOICE #4096, JOB NO.: 4027 Pump needs fixing on a heat recovery unit B-pod. Leak will need to be repaired, pump seal replaced after running dry and motor starter that had previously failed. System has chemical in it and will need to be added after repair work. Daniel Evans talked to Paul and he will not carry out any repairs on this at this time. THIS IS A PREVAILING WAGE PROJECT. An approved (by Labor & Industries) Intent ($40) to pay Prevailing Wages is required to be submitted to this Office prior to first payment, and an approved Affidavit of Wages paid ($40) is required prior to final payment for projects over $5,000.00. A Combined Intent & Affidavit Small Works Contract (FREE) is required for projects $5,000.00 or less.
